Your soccer team has signed up for one of our free team training sessions. Now we need each parent/player to fill out the simple form below in order to comply with our insurance policy and facility regulations.

You will get an email that provides out insurance waiver form which we need everyone to fill out and bring to training sessions as well as information about what to wear and how to find our facility.
